Add the ability to disable fallback #61

@frenck Is fallback a good name? It would default to true to keep the current behavior and could be set to false to disable fallback. Or should this be inverted to something like nofallback? Is the DNS server selection logic good?

@frenck The second commit just makes the forward line easier to read. There is no change in behavior. But I can revert if you would prefer.
