Many missing explanations of components/classes in DG #15

Only the Add Command, Retrieve Command, Edit Command and Delete Patient Command have sequence diagrams and brief explanations. The rest of the command classes are not even mentioned/do not have a brief explanation in the developer guide.

Similarly, there are no explanations for the components as well.
