Get rid of Identifier vs. Reference distinction #207

A long time ago, we added class Identifier as opposed to having, say, a
bare String value for the symbol bound by a Declaration. This created a
dichotomy between References and Identifiers that causes much pain and
conversions. Can we just have one type of node for both usages?
