fromHaxml does not handle entity-refs. #15

e.g &lt; &gt; will cause it to break.  &amp; is hardcoded in.

Not sure of a good solution given that technically this means doing IO to 
retrieve the DTD.  Unsafe retrieveal of the dtd is probably ok.  as is 
hardcoding a decent set of the standard refs.
