Update markdown in Changelog and Readme files. #11

Hi @horst-n,

Thanks for your ever-useful module! I've made a few changes that might help regarding the Markdown formatting in the Readme and Changelog files...

  1. Updated the Markdown in the Readme file to correctly format the headers.
  2. Updated the Changelog to allow ModuleReleaseNotes to detect change regions based on version numbers.
