Add support for footnotes and endnotes.

The solution should support placing the notes at the end of the volume, 
structured by page number. It might be possible to extend the toc feature to 
support note placement relative to the volume.

Support for placing the notes at the bottom of the page is not needed now, but 
should be possible to add in the future.
