Break into volumes while paginating #93

Some features require that the current volume is known when paginating, e.g. 
when continuing the page numbering in post-content volume material. This can be 
solved by breaking into volumes while paginating. Doing so would also be more 
efficient, as cross references may be resolved sooner and data structure 
copying can be reduced.
