Remove click from output when toggling plugin bypass #2

The feed to the up sample routine is not continuous when the plug-in is bypassed producing a glitch (click) in the output. Need to flush the output buffers when deactivated and prime the input to the up sample routine with a block of zeroes.

The input and output buffers are now flushed and the input buffer primed with silence to prevent audio left over when the plugin was bypassed from being output when the plugin is then enabled.
