Cosmetic updates for request and task forms.

@sharkinsspatial Nice, 👍 Tested and works well. Seeing one exception in Chrome and Firefox. The search results are hidden below the map controls:

@sharkinsspatial apologies, that above comment is also about the other PR #146, just had tested this branch first.

Couple other questions:

The future date selection for the Desired Time Frame on the Request creation/edit view doesn't allow me to select in the future. It seems to work in the Task Edit view.
Can we add some space and help text to the Task edit form so it is more in line with the wireframe in #131? Unless you tried this and it didn't work well.
