Log user activities #1921

This came up several times. It would be good to track user actions to know who did what on the system. Especially when it comes to assign user roles, or creating other users it can be helpful to understand more about the context.

Besides actions of users, @russdeffner can you think about other parts that need more logging?

As recently came up; logging user roles needs to include user blocks so we can identify if someone was blocked for a reason or accident if they we suddenly get a question from them as to why they can't take any actions on the TM.
