Create executable bug report Rails application #593

This template draws inspiration from Rails' bug report templates.

@jorgemanrubia it'd be beneficial to pair this script with a .github/ISSUE_TEMPLATE.md file like Rails uses. There is a new version of GitHub issue templates that could be worth investigating.
