error handlers not working according to sessions proposal #21

The sessions proposal allows for some use of MPI error handlers prior to calling MPI_Session_init. The prototype 'C' interface is nominally adhering to the standard, but the Fortran entry points are not owing to the use of an internal variant of the error handler f2c function.
