Remove qv_policy_t. #187

Remove qv_policy_t since it doesn't appear to be used.

@samuelkgutierrez : I left it in there intentionally. I had a discussion with Edgar today about what I wanted to do with it and I plan to discuss this on Tuesday's meeting.
