fixes #656 split personal information into school information and teacher information

ask for school address, not teacher address, name change in database

text states mobile number is wanted

ask additionally for the grade of the pupils int the workshop information section

in the label next tot the field asking for the knowledge level, it states that it should be descriptive, also it is bigger so that it looks as if you should put at least 3 lines in
