Add support for CommonMark with extensions #50

As discussed in https://github.com/hplgit/doconce/issues/48 , support for both-way translation between DocOnce and CommonMark with extensions would be nice as the extensions are already getting into the most popular CommonMark implementations (e.g. https://github.com/markdown-it/markdown-it).

It would be nice, and it should be quite straightforward, but I don't have capacity at the moment to increase the support for CommonMark.

Understood, no hurry - I just wanted to make a public note, so that anyone with few free hours could try that.
