Per-output enablement
Maximum time
Enable only when output is inactive

Closes #66.

I've had this running for a few hours at my desk, with the screensaver disabled on output B, enabled on output A with a maximum time, and disabled on output A when it is not the active output. It has behaved the way I expected it to!

Awesome, thanks again for working on this! Let me try it out first and will merge, then I have to resolve some merge conflicts (because I was working on multi-desktop workarounds). There shouldn't be much overlap.
