IANA table columns #1037

<!--[rfced] Section 16: The subsections of Section 16 give information
     on the various IANA registries.  We had the following questions
     related to these sections as a whole:

a) We note that the fields listed as being "MUST"s do not exactly
match the column headings in the corresponding registries.  Might it
be helpful to the reader if they did?

b) We see that the "Notes" column listed in the IANA registries (see
below) is not mentioned in Sections 16.5.1 and 16.6.1.  At first, we
assumed this intentional as it would not be a "MUST".  However, we see
that Section 16.3.1 includes some "optional" information, so please
let us know if any updates are necessary.
