use of RFC2119 in introduction #515

In P5's and P7's introduction sections, we use "OPTIONAL" to say that either the whole feature optional (P5), or that some aspects are optional (P7: support for specific schemes).

Proposal: in both cases just lowercase the "OPTIONAL" and be done with it.

We occasionally use RFC2119 keywords before they are defined.

Need to decide whether this is a problem that needs to be fixed.
