Add WIKI documentation which describes problems with WSGI specification. #65

The intent of this documentation would be to list all the areas of the WSGI
specification which are unclear, ambiguous, incomplete, not logical or
possibly just plain wrong. The point of doing this would be to drive the
release of amendments to 1.0 specification, a new 1.1 specification, or
serve as input to proposed incompatible 2.0 specification.

One hopes that it isn't too late to fix things and end up with a better
specification rather than one that works for mainstream cases but has
problems when one goes beyond that.

Closing out documentation issues. Will pick up another time on github side when 
get enough momentum to work on documentation again.
