Refact all terms reports #283

Instead of having one entry for each row, now the reports show the list of rows for the same issue.

For example, in the Blank Ontology ID report:

  1. No term id was found for the name/label intramural segment of Fallopian tube in the following 24 rows: 12, 13, 14, 15, 16, 17, 18, 19, 20, 21, 22, 23, 24, 25, 26, 27, 77, 78, 104, 105, 106, 107, 121, 122.
