Create AVR FAQ, Update AVR Upload page text, upload 4 templates, update csv-format.md #61

  1. Add avr-faq.md Frequently asked questions page for AVRs
  2. Update (avr-upload.md), AVR Upload page text -Correct some typos: antibody validataion reports (AVRs) to antibody validation reports (AVRs) can be fount to can be found -Rephrased: The associated antibody validation reports as PDFs. to The associated antibody validation reports saved as PDFs.
  3. Update [csv-format.md] HuBMAP Antibody Validation Report CSV Structure table to match new AVR csv template; we added 11 new fields with descriptions to the avr template (omap_id, cycle_number, fluorescent_reporter, antigen_retrieval, HGNC_ID, tissue_preservation, concentration_value, dilution, conjugate, manuscript.doi, isotype and removed 1 field (antibody_name)
  4. upload these 4 templates, renamed so they substitute for the existing templates without having to redo any calls from other pages. avr-template.csv, example-avrs.csv, example-avr.pdf @shirey I'm not sure where it would be best to link to this template to download so people could have a form to fill out to generate their AVR PDF? avr-template-form.docx the NEW template (there was not a word doc to fill out for producing the PDF online)

@emquardokus I've created a separate branch to enable changing file names while keeping the old versions around. I'll close this PR and delete this branch.
