Can you try to collect some logs. See HowToCollectLogs wiki page.
You can also try :
* install latest nightly build : http://nightlies.csipsimple.com/trunk/
* Activate STUN as explained here : http://code.google.com/p/csipsimple/wiki/FAQ?wl=en#The_other_party_can_hear_me_but_I_can_not_hear_them
* Reduce number of codecs enabled in the app (settings > codecs). If your connection has a bandwidth fair enough, PCMU/PCMA can be good to start test cause it is usually the most supported codecs. If bandwidth is too low (3G for example), you can try with other low band codec supported by your sip provider.
* Can also be something with the manufacturer's audio driver. Sometimes, they do weird things and you have to find the correct setting ito get the app working. This is explained here : code.google.com/p/csipsimple/wiki/FAQ?wl=en#Audio_routing_troubleshooting
