Require separate domain and server fields

My SIP provider requires a separate domain and server addresses. 

I had a quick hack and added a domain field, using this for the realm,
while the server was only used for the remote IP. This actually worked, but
no guarantee I've not missed something.

ie:

Your Phone Number: PHONE_NUMBER
Your Phone Password: PASSWORD
VoIP SIP Domain: iinetphone.iinet.net.au
VoIP SIP Server: sip.qld.iinet.net.au
Preferred Codec Order: G729, G711ulaw, G711alaw
SIP Re-registration Interval: 300 seconds
DNS: Use SRV records
