async mapping #10

fixes #8
uses node-style callbacks, e.g. done(err, result)
has tests for passing and throwing errors
updated readme to reflect async options
added concat-stream as a dev dependency, since it's required for tests and wasn't in the deps before
only needs a minor release since it doesn't break any functionality (⌐■_■)

I totally forgot about this, since I've just been using my fork. :stuck_out_tongue:

This should definitely be merged if you have the time, @hughsk :+1:

hey @hughsk is this library still being maintained? (I wouldn't mind helping maintain it if you're busy with other things)

Because of looming issues with bl and npm v3, I've decided to fork this. I added in async support, and I'll go through the current issues and PRs to see if there's anything else I should bring over.
