Add a plugin icon #1

Would be nice to have a plugin icon so that we look nice in search results.

No assets directory in this repo for them to go, but you can use these if you like:

Icon 128x128

Icon 256x256

Banner 772x250

Banner 1544x500

@saschamt @susandyoung are you happy to go with the above icons for this existing version? Also happy for you to provide icons and an updated header graphic if you'd like it to use this as an opportunity to change them.

There are simpler icons posted on the other thread. I had also said to provide new banner images tomorrow
