add two more levels of subheading #127

I'm writing a novel that needs more than two levels of headings, so I added two more levels. Rather than go with "subsubheading" and "subsubsubheading" I named them "heading3" and "heading4". In addition to parsing them for links I added styles to indent them further in the navigation pane.

It's a good idea, I can implement it. Your PR changes the code and css formatting, I cannot merge this.

Ah, sorry, my editor reformats things automatically and I forgot to turn that off.
