Is clean and neat? (yes/no/na): no Assessment/Observation/Comment: bad odor, no fan or ventilation wet, sticky, dirty floor, trash can over filled… all over the floor, markings, graffiti on the wall...

Physical Structure

Any damage or brokenness? (yes/no/na): no Assessment/Observation/Comment: holes in the walls sink is sagging soap container lid missing, not working towel dispenser is not working faucet not working, water barely flows

Safety Concerns

Electrical, Mechanical, Ergonomic (yes/no/na): yes Assessment/Observation/Comment: wet floor can be slippery and causing fall & injury electrical outlet without cover, wires exposed suggest to replace the floor with material has rougher surface, replace toilet and sink altogether

Hello all, this is to clarify that the Issue titled "Old Man Restroom" Hung posted here is just an example I had made up for demonstration of the form use. It is not the real report from the walkthrough. Hung will need to replace it with the real report. The report format is good and I like it. Thanks, Dinh

Hi, I think we should keep this one as a template for people to copy and paste when they create new issue.

Well, I have converted this issue into an actual issue. I think we should replace floor, toilet, and sink all together.
