Cleanliness & Neatness

Is clean and neat? (yes/no/na): yes Assessment/Observation/Comment:

Physical Structure

Any damage or brokenness? (yes/no/na): yes. Water leak on the roof and ceiling towards the church wall Assessment/Observation/Comment: Need roofing contractor to repair the flashing between roof and wall of the Blessed Sacrament chapel and gift shop.

Safety Concerns

Electrical, Mechanical, Ergonomic (yes/no/na): yes. On the rain and windy day, There will be a lot of water on the ceiling, wall and floor. If the 120V AC hot wire expose and short to wet material, people may get electrical shock. The ceiling and wall structure may get farther damage if it expose to falling rain water.

Assessment/Observation/Comment: Please have roof repair as soon as possible to minimize the cost.

We need insurance because we don't know how safety the celling is if we step on it or fix the proof. We need to cut the celling to see damage. It been there for at least 2 years. It is not even safe for people to walk in and pray. We need to call inspector from diocese.
