Name of Inspector: Dean Corpuz, Lina Simpliciano, Philip Building: Trinity Center Building Room or Area: Men's Restroom Inspect Date: January 14, 2018

Assessment/Observation/Comment**

Handicap Stall toilet need to be tighten Middle Stall Lock at Door is missing. needs to be replaced

Plan of Correction

1st Urinal needs to be replaced, P-trap needed Partition is loose need to fix partition Paper Towel dispenser is loose so tighten screw Ventilation needs to be tested

1st Urinal was replaced, and P-trap is fixed.

Action:

  1. Order 2 new partitions, crews, and nuts. Need to measure the size.
  2. Ventilation needs to be tested again and add motion sensor for the fans.
  3. Paper Towel dispenser is loose so tighten screw: need key to inspect if it is working correctly.
  4. Handicap Stall Toilet, Paper Towel Dispenser, Door Middile Stall Lock... Dean and Thien
  5. Restroom floor cleaning
