This PR refactors the replicator.

[x] Updates the merkle tree proof/verify to use an unambigious format (block, hash, seek, upgrade) vs (blockOrHash, seek, upgrade)
[x] Moves to use Protomux (simplifies multi version support)
[x] Adds timeout/ifavailable support
[x] Replication snapshot support
[x] Replication cancellation support

@kasperisager @andrewosh ready for review (the replicator2, protocol2 stuff) - i'm fixing the last parts now that just include hooking it up to index.js and making the tests pass, which is pretty easy but would love for y'all to just go through this as is.
