Detect connection issues between nodes #16

When nodes report each other as unreachable upgrade the connection issue to an error status.
Update the format of the unreachable node warnings, use the same format as the validator-info results.

I agree, I'm going to take another pass at summarizing the unreachable nodes and count in the --status summary more concisely.
