Update dashboard header to be closer to designs #6372

Depends on https://github.com/hypothesis/lms/pull/6376 and https://github.com/hypothesis/lms/pull/6377

Remove the centered top logo in the dashboard's header, and add a text-less brand icon to the left, with a settings link to the right.
Replace img tags pointing to a local svg logo, and use LogoIcon from frontend-shared instead.
Add active user's name and settings link.
