Additional Paths for routine care variables.txt

Additional paths to be added to the xref table for Flu Shot, Pneumococcal vaccine, Mammography, Fecal occult , PSA and Colonoscopy Updated path for pneumococcal vaccine per Darren's comment

I tested the paths suggested. 15/16 return results from our version of the ACT V2_2018AA ontology and I confirmed the concepts are not currently being pulled by the other paths.

The new path suggested for Pneumococcal vaccine '\ACT\Procedures\HCPCS\V2_2018AA\A13475665\A13296622\A18775666\A16024072\A23608269\CG0009\' was not found, but a path one c_hlevel above it at '\ACT\Procedures\HCPCS\V2_2018AA\A13475665\A13296622\A18775666\A16024072\A23608269\' does return the concept 'HCPCS:G0009'

I don't think our tables are out of compliance with the network.
