I came here for the same reason. Distant Horizons has a GitLab issue for their end at: https://gitlab.com/jeseibel/minecraft-lod-mod/-/issues/94. I've added what I've uncovered from my logs to their issue.
I suspect compatibility to be most easily fixed by having their end either stop distant rendering when player is near a portal, or at least stop rendering distant chunks for the opposite side of the portal from the player (not sure how that would work in multi-player if players are on both side of the same portal...)
Anyway, it may still take effort on the part of both mods to rectify the problems.
I'm also interested in the combination of these two mods. I'd be totally satisfied if distant horizons only works in the current dimension and the portal world is limited by the normal rendering distance.
Not sure if I can contribute anything, my knowledge of both Minecraft and (portal) rendering isn't good enough.
