remove dead link in client-management README
Markdown formatting for touched READMEs

Pulled the repo and noticed a couple of possible readme issues. I didn't notice any clients still using the v1 endpoint, but let me know if I was too eager here.

Also, the app at https://itwindemo.bentley.com/ does not appear to be alive, so I've removed that reference.
