Missing #define Foo_tag for extension fields #93

What steps will reproduce the problem?
1. Build the test in tests/extensions
2. Note that extensions.pb.h doesn't include tags for AllTypes_extensionfield1 
and ExtensionMessage_AllTypes_extensionfield2.
