Allow .pages file at top level as well as in subdirectories #91

There's a side effect is that pages files could in principle override "collapse-single-pages" directive. I don't think this particularly matters.

After merging a whole bunch of other stuff this PR had some merge conflicts and I didn't want to have to battle git, so I added the changes manually. Thanks! :)
