Update info about Spreadsheet question format. #21

Add a sample quiz template. Update text in Readme and add an image. Update text in modal specifying file formats as well as spreadsheet format. FIX #13

Changed some readme text from excel file to spreadsheet Changes can be seen here. Overall file can be seen here
Add sample quiz file
Changed Upload modal text to specify some things:
file_upload page now shows this text

instead of blank screen. (This change was made because, in case of any error (ex wrong spreadsheet format) the application used to get stuck on this page without any instructions.)
