Simplify adding new networks

There's no standard way to add a new network and it should be simpler if possible.

List of possible things to address

• parameters in bitcoinjs-extensions.js
• parameters in segwit-parameters.js
• a test for the generated address
• an entry in index.js
• any unusual situations in index.js for address or key formats (eg ethereum).
