MIDI off note missed #2

Experimenting on keyboard occasionally note off is not received and the note is left hanging.

In general a "note on" can be ignored if too late, however a "note off" MUST be reliably transmitted even if a little too late.

Closing, various bugs around note reliability have been fixed. I suspect this is fixed, but if it occurs we can reopen with specific details.
