Fix a number of issues and add in some new features #29

  1. I've rearranged the docs. There was some duplication, and I've significantly simplified the structure.
  2. Fixed a couple of issues.
  3. It would be very helpful to add the instructions to judges at the top of each judgement page. If you don't agree, then I'll add in an option "show judgement instructions at the top of the judgement page" so people can choose. George and I both agree we want it!
