As a doctor I would want to create diagnosis report for my patients to better serve them

Acceptance criteria : 1.Doctor can create new report based on selected individual patients

  1. Doctor can edit the existing document if he found errors that needs to be changed
  2. System can save doctor reports in the database
  3. Doctor must be able to type reports on his system for the report to be written
