pull interrupt enhancement from master up into capi2 branch #97

enhancements to interrupt process in master need to be merged into capi2 also an update to the test afu makefile needed to be pulled in.

Most conflicts were straightforward. 2 were not. The makefile in test/tests had, I think, the wrong location for cxl.h. and cmd.c around line 2320. I had to pick code from both master and capi2.
