The index link is broken in some cases #45

The index link is hardcoded and won't work from inside a package. See http://ibm-js.github.io/delite/docs/api/0.5.1/delite/KeyNav.html as an example.

Also reproducible with jsdoc-amddcl's own sample project (https://github.com/ibm-js/jsdoc-amddcl/tree/master/sampleprojects). After navigating to the doc of any module, click for instance the Index link (in the right-side TOC) => 404.
