Error Getting Form List

What steps will reproduce the problem?
1. Get Blank Form

What is the expected output? What do you see instead?
Expected: List of Forms on Aggregate Server
Get:
Error Getting Form List
Form listing failed. Error:
libcore.io.ErrnoException: getaddrinfo failed:
ECONNREFUSED (Connection refused) while accessing http://[doman].com/formList.

Downloaded May 2014 on Android.

Please provide any additional information below.

I was successfully able to add forms to three devices, when I got the fourth, I 
got this error. Now, when I go back to the first three devices where I already 
successful downloaded the forms, they give the same error and cannot access 
additional forms.

The ECONNREFUSED indicates a server or network error of some sort. 

Were you connecting via wi-fi? 

Or could you have exceeded your cellular data transfer limit?

If this occurs often, open a browser on your Android device and navigate to 

http://[domain].com/formList

What do you see?

What ODK Collect version are you running?

What Android version are you running?

Has this issue been resolved? Seems I am facing the same with ODK Collect 1.4.5 
(1048)

It has been working fine the one day and the next day when I wanted to connect 
to a different server I got this same error.
