Converting xls to xform not working #1098

What steps will reproduce the problem?
1.go to https://opendatakit.org/xiframe
2. browse for xls file wanting to convert to xform, Click 'Submit'
3. Verify no errors (by previewing in enketo). 
4. Click 'download' so can convert
5. No converted file is created. Instead a field containing the data field info 
is presented, but no where can I can save/download/create as xform file.

What is the expected output? What do you see instead?
An xform file. No created file.

What version of the product are you using? On what operating system?
I tried on Win 7 and Win8. It was previously working for me.

Please provide any additional information below.
