display `cei:index/@ref` as link in single charter view #595

attribute introduced by request of project Santa Maria della grotta to allow reference of terms in the text to external sources (see #593, commit f0d8f2a). In compliance to http://www.tei-c.org/release/doc/tei-p5-doc/en/html/ref-att.canonical.html .
