Code snippet fixes #602

This PR fixes several issues surrounded snippets and handling of UTF8 characters:

The lexers now handle UTF8 characters when advancing though their buffer.
We no longer try to compute the line indices up front when we parse the slice file.
Line snippets which only match the end of the line are no longer emitted

This fixes #589
