There is no template in microsoft power automate to create a flow that creates a github issue from a forms answer, but it is possible to do. I'm looking at this and will be testing this in the coming days/weeks (depending on how well it goes). I'm not sure if it's possible to assign the issue to a specific project though. I'll keep looking into this. Expect some test responses to the form + github tickets coming our way.

I'm able to create an issues from a microsoft forms response, but I'm unable to automatically assign the issue to the RMG project, though I don't think that's a major problem. I also created a flow with an email receipt to the requester. The email is CCing to the accessions mailbox as well, so that we can move the issue to the RMG project board.
