SAM does not handle (completely) missing years gracefully #33

What steps will reproduce the problem?
1. Remove all data from a year in the middle of the assessment e.g. NSAS 
assessment, remove 1960 catch data
2. Run assessment
3. Assessment fails

Problem lies in FLR2SAM, in the way that it writes the year indices
